# Security exceptions and risk acceptance

When a legacy production system cannot be patched immediately, a note saying accepted is insufficient. Make the service, threat, impact, compensating controls and decision authority visible. This guide connects exception requests to risk acceptance, reassessment and validated closure.

## What should I do tomorrow?

1. Identify the exact unmet requirement and business owner of one gap.
2. Test the compensating control and describe residual risk.
3. Record authority, expiry and early-review triggers.

## Concepts

**İstisna** — A bounded deviation from a defined control or policy requirement.

**Risk kabulü** — An authorized business owner’s decision to bear defined residual risk.

**Telafi kontrolü** — An alternative measure reducing an unmet control’s impact, requiring validation.

**Kalan risk** — Remaining uncertainty and possible business impact after measures.

**Risk iştahı** — Management direction on risk the organization is willing to bear for its objectives.

## Process map

### 1. Describe deviation

Control owner

Record unmet requirements, targets and reasons.

Scoped request.

### 2. Assess risk

Business owner + security

Assess threats, impact and compensating controls.

Residual-risk analysis.

### 3. Obtain authority

Risk authority

Approve or reject duration, boundaries and conditions.

Reasoned decision.

### 4. Monitor and escalate

Assigned owner

Monitor conditions and reopen on risk changes.

Monitoring and notifications.

### 5. Close or renew

Business owner + validator

Test remediation; treat renewal as a fresh decision.

Closure or new decision.

## Distinguish exception from acceptance

A policy deviation does not automatically accept risk. Specify control, assets, services and time. Evidence should support technical, cost or operational constraints. An unexplained business need hides alternatives and consequences.

Internal exceptions do not automatically waive legal, regulatory or contractual duties. Obtain appropriate legal and authorized assessment where such constraints exist. Teaching records in this guide do not issue organizational approval or compliance certificates.

## Describe risk as a business scenario

For a fictional unpatched server, distinguish actor, access path, weakness, affected transaction and outcome. A high-risk label alone cannot support a decision. Record evidence, assumptions and uncertainty instead of inventing precise probabilities.

Removing internet exposure may reduce external attacks but not compromised internal identities. Compare scenarios before and after controls. Assess service dependencies, sensitive data and recovery time together, connecting technical weakness to business impact.

Decision and evidence boundaries

| Subject | Focus | Validation / boundary |
| --- | --- | --- |
| Exception request | Deviated control and target | Request is not approval |
| Risk acceptance | Authorized owner and residual impact | Does not automatically waive law or contract |
| Closure | Remediation and effectiveness test | Uploading a document alone is insufficient |

- [ ] Assumptions, evidence and remaining uncertainty are separated.

## Test compensating controls

Segmentation, narrow access, application allowlisting and added monitoring are measures only when applied and observed. Assign owners, scope, tests and failure signals. A screenshot does not demonstrate sustained operation; validate at suitable intervals and after change.

Assess whether compensating measures interrupt service or introduce other risks. Coordinate OT testing with operational safety. Predetermine when a failed measure suspends the exception and whom to notify.

- [ ] Compensating controls have operating and failure evidence.

## Who may accept which risk?

Organizations define decision authority by service and impact. A technical operator cannot alone accept all financial or regulatory exposure. Security supports analysis; the authorized business owner owns the decision, resources and deadline.

Decisions specify scope, reason, alternatives, residual risk, conditions, expiry and triggers. Lack of funding is not automatic indefinite acceptance. Escalate impacts requiring higher authority; copying someone on an email is not approval.

- [ ] Authority, decision and resource responsibility align.

## Expiry and changed conditions

Expiry needs visible reminders, deputies and overdue escalation. New exploitation, exposure, increased data sensitivity or compensating-control failure may trigger early reassessment. An expired record must not silently remain active.

Renewal is not copying old approval. Reassess remediation, delays, current threats and new deadlines. Report repeated extensions together: temporary exceptions may have become structural debt. Consider replacement or retirement when appropriate.

- [ ] Early-review and expiry actions are defined.

## Closure evidence and management visibility

Validate both control implementation and reduction of the scenario. For a patch, check version, service function and relevant access paths. Removing compensating controls can create change risk; authorize closure and control removal separately.

Report services, age, overdue status, residual impact and repeated extensions alongside open counts. Aggregate exceptions sharing dependencies for systemic decisions. A closure percentage alone should not encourage hidden records or evidence-free closure.

- [ ] Closure includes test results and related-control decisions.

## Common mistakes and better approaches

**Leaving exceptions indefinite.** — Assign expiry, deputies and overdue escalation.

**Not testing compensating controls.** — Validate scope and failure signals at the real target.

**Leaving all risk acceptance to technical teams.** — Assign authority appropriate to business impact.

## Decision examples and explained answers

### A signature exists

A manager signed, but a contractual requirement remains unmet.

**The exception waives everything.** — Revisit

Internal approval does not amend external obligations. Contractual exposure and necessary customer/authority processes require separate assessment.

**Assess the contract and authority boundary with responsible specialists.** — Supported approach

Keep the deviation visible while legal and business authority assess duties, options and needed agreement. Record conditions within actual authority.

### Control failed

Segmentation protecting an unpatched system was removed.

**Wait until expiry.** — Revisit

A condition supporting acceptance changed. The old residual-risk assumption is no longer established simply because expiry has not arrived.

**Reassess the exception and escalate risk.** — Supported approach

Notify ownership and validate exposure and impact. Authority chooses temporary control or suspension, recording reassessment and timing.

### Third extension

A temporary exception is being renewed for the third time.

**Copy the old approval.** — Revisit

Repeated extension can normalize temporary controls. Threat and cost assumptions may have changed; a fresh decision requires current evidence.

**Assess current risk, alternatives and structural remediation.** — Supported approach

Escalate remediation barriers and alternatives to the owner and clarify resources. Any renewal receives fresh authority, conditions and dates, with repetition visible in reporting.
